The following services are available within our VNA Service Area.

Skilled Nursing

    Nursing care in the home is one on one. Care includes assessment, treatment and teaching related to new medical diagnoses or after surgery, illness or injury. Our staff provide emotional support while teaching patients and family members how to manage medications care and treatments at home.
 


 
Rehabilitation Services
    Physical Therapists, Occupational Therapists and Speech- Language Pathologists work with patients in the home to achieve maximum function, independence and quality of life. Patients with neurological disorders or weakness or people who have recently undergone orthopedic, cardiac or other types of surgery may benefit from these services.

Home Care Aide

    Home Care Aides provide personal care assistance for short periods during the recovery and family teaching process.

Social Work

    Our medical social workers provide counseling around disease adjustment and care planning processes.

Specialty Programs

    The Greater Medford VNA has a Certified Wound Ostomy Nurse and a bridge to hospice program. These specialties offer our patients a higher level of expertise and care. We also have program specialties in maternal/child health and heart failure. Feel free to call us for more information about these programs.
