Community Liaison (Greater Medford Area) - Full Time Overview Full-time position includes a highly competitive compensation and benefits package. Successful candidate will assist in the planning, coordinating, and evaluating systems for effective and efficient referral and admission of patients to the Greater Medford VNA. Responsibilities Core responsibilities include developing and implementing a community relations strategy and maintaining strong relationships with physicians and referral sources. Requirements Qualified candidates will have two to three years relevant experience in home health care; discharge planning, liaison or community relations preferred.
